On or about January 17, 2023 and times prior there to, employees were not protected from flying debris and airborne particles via safety glasses or face shields.

On or about January 17, 2023 and times prior there to, all working levels of the scaffold system occupied by employees were not fully planked between the uprights and guardrail supports to create a complete working surface.

On or about January 17, 2023 and times prior there to, employees were working from a multi-tiered scaffolding system that's vertical support poles were not bearing on a solid base of mud sills or base plates while occupied by employees.

29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.   On or about January 17, 2023 and times prior there to, employees were not provided with a safe means of egress from the multi-tiered scaffolding in the form of an attached ladder or other safe means.
