At the shipping and receiving warehouse, located at 7660-20 Phillips Highway, in Jacksonville, FL; On or about January 19, 2023, and at times prior, the employer exposed employees to struck-by and crushed-by hazards, in that the employer did not ensure that each Powered Industrial Truck (forklift) operator had been trained and evaluated as required by this paragraph (l), prior to the employee's operation of the truck.

At the shipping and receiving warehouse, located at 7660-20 Phillips Highway, in Jacksonville, FL; On or about January 19, 2023, and at times prior, the employer exposed employees to struck by hazards, in that Power Industrial Vehicle, Yale Veracitor 30VX, was not examined by operators before being place in service.
