29 CFR  1926.451(c)(2)(v):The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the forks:    a.  At the residential construction located at 1100 Tallacoe Place in New Smyrna Beach Florida, 32168: On or about January 24, 2022, and at times prior, the employer exposed employees to fall hazards, in that the employer did not ensure the entire scaffold platform was attached to the forks prior lifting personnel 16 feet  to the first floor roof.

On the roof at the jobsite located at 1100 Tallacoe Place in New Smyrna Beach Florida, 32168: On or about January 24, 2023; The employer exposed employees to fall hazards greater than six feet in that they are not requiring the use of personal fall protection while working on a steep pitched (8/12) residential roof.
