29 CFR  1910.178(q)(7): Industrial trucks were not examined before being placed in service:  a)231 Executive Drive Newark, DE: On or about January 23, employees operated a Crown Powered Industrial Trucks[ PIT] to move pallets of equipment around the warehouse with conditions that adversely affected the safe operation of the vehicle including but not limited to, inoperable lights, back-up alarms and a missing seat belt. No examination was made which would have identified the  service needed of  the PIT .

29 CFR  1910.303(g)(1):Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  a) 231 Executive Drive Newark, DE 19702: On or about January 23, 2023, employees  were exposed to electrical hazards due to the access and working space about electrical charger of the powered industrial trucks , including but not limited to  make/model Exide E58-18-680, 36 volts/109 amps serial number HRS58706-2HK  having improperly stored combustible materials [e.g. wooden pallets and cardboard boxes] within three (3) feet of the panels.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  a) 231 Executive Drive Newark, DE:  On or about January 23, 2023, strain relief devices were not provided for the flexible charging station cable at the battery charging station, where employees regularly charge the powered industrial trucks, which resulted in employees being  exposed to the energized cable. The make is an Exide Model number E53-18-680, 36 volts and 109 Amp DC.
