29 CFR  1910.30(a)(3)(iv): The employer did not train each employee in the correct use of personal fall protection systems and equipment specified in paragraph (a)(1) of this section, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer.  On or about January 25, 2023 and at times prior thereto, where employees who were working on telecommunication towers were not provided hazard recognition and avoidance training and the knowledge to understand the difference between the synthetic rope grab and cable grab (safety Assists) for fall protection systems, exposing the employees to fall hazards.

29 CFR  1910.268(e):Personal protective equipment, protective devices and/or special tools needed for the work of employees were not carefully inspected by a competent person to ascertain that they were in good condition:  On or about January 25, 2023 and at times prior thereto, where a competent person did not inspect the safety grabs (safety assists) prior to allowing employees to climb the communication tower and to ensure the appropriate cable grabs for the work were used for climbing communication towers, exposing employees fall hazards.
