29 CFR  1910.305(g)(2)(iii):Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws.  On or about May 04, 2023, flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws on a flexible conveyor being used at inbound dock door 228.  These condition exposed employees to electrical shocks.

29 CFR  1904.7(b)(4): The employer failed to enter the correct number of calendar days for a work-related injury on the OSHA Form 300 or equivalent.   OSHA 300 log 2023, Case No. 2 (Pick to Rebin) occurring on 1/19/2023, an employee injured their right shoulder after pushing an item into the bin. The injury resulted in restricted work activity that began on 2/15/2023 and records showed that the employee was scheduled for a follow-up visit on 03/28/2023. The injury was recorded with only 7 restricted workdays (started from 2/15/2023 thru 02/21/2023) even though there were at least 42 or more restricted workdays: the record does document that she was last scheduled for a follow up visit on 3/28, having been on modified duty (with physical therapy and lifting restrictions) from 2/21 through 3/28.
