29 CFR 1926.502(d): "Personal fall arrest systems." Personal fall arrest systems and their use shall comply with the provisions set forth below. Effective January 1, 1998, body belts are not acceptable as part of a personal fall arrest system. Note: The use of a body belt in a positioning device system is acceptable and is regulated under paragraph (e) of this section. a. Located at 157 Kohler St. Pickerington, Ohio: On or about January 31, 2023, the employer did not ensure employees were protected from falls while clearing ice and snow from the residential roof to prepare for the installation of roofing. Employees were using unapproved climbing harnesses as personal fall arrest equipment, thereby exposed to falls from approximately twenty feet.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows: a. Located at 157 Kohler St. Pickerington, Ohio: On or about January 31, 2023, the employer did not ensure that each employee was provided with an approved anchorage point for the attachment of personal fall protection equipment. Employees were using unapproved climbing harnesses as personal fall arrest equipment that was attached to a rope lifeline wrapped around the roof truss and nail plate and tied to itself, thereby exposing employees to falls from approximately twenty feet.
