29 CFR 1910.147(e)(2)(i): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not ensure that all employees had been safely positioned or removed from the work area:  a)	Granny Room Area: On or about October 28, 2022, and at times prior, employees were exposed to unexpected machine startup while conducting maintenance on the Granulator Air Lock. The employer was brought in to isolate and control electrical power on the equipment and applied and removed their lock without notifying authorized and affected employees.

29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures:  a) Granny Room Area: On or about October 28, 2022 and at times prior, employees of American Crystal Sugar DBA Sidney Sugars, Inc. were exposed to unexpected machine startup while conducting maintenance on the Granulator Air Lock. As a subcontractor, Prairie Electric Inc., was brought in to isolate and control electrical power on the equipment without informing Sidney Sugars, Inc. of their respective lockout or tagout procedures.
