OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards as a result of steel shelving storage racks structural collapse in the following areas:  (a)  Employees loading / unloading pallets of materials  into steel storage racks in the warehouse section of the facility on the north side of zone (aisleway) 6A where steel shelving units had not been periodically inspected for corrosion, structural damage, or overloading.  Column posts 1 through 7 were out of alignment creating an unstable storage rack unit.    (b)  Employees loading / unloading pallets of materials  into steel storage racks in the warehouse section of the facility on the north side of zone (aisleway) 7A where steel shelving units had not been periodically inspected for corrosion, structural damage, or overloading.   Column posts 1 through 7 were out of alignment creating an unstable storage rack unit.   (c)  Employees loading / unloading pallets of materials  into steel storage racks in the warehouse section of the facility on the north side of zone (aisleway) 8A where steel shelving units had not been periodically inspected for corrosion, structural damage, or overloading.   Pallets of material were being supported by storage rack bracing that was not attached between column posts 6 and 7, thus creating an unstable storage rack unit.   (d)  Employees loading / unloading pallets of materials  into steel storage racks in the warehouse section of the facility on the north side of zone (aisleway) 9A where steel shelving units had not been periodically inspected for corrosion, structural damage, or overloading.  The back side of column post 1 had structural damage with pallets of materials being stored in the storage racks.  Column post 7 floor base plate was not touching the floor.  (e)  Employees loading / unloading pallets of materials  into steel storage racks in the warehouse section of the facility on the north side of zone (aisleway) 10A where steel shelving units had not been periodically inspected for corrosion, structural damage, or overloading.  The front side of column post 1 had structural damage with pallets of materials being stored in the storage racks.   Column posts 2 through 7 were out of alignment creating an unstable storage rack unit.   Column post 7 floor base plate was not touching the floor.  (f)  Employees loading / unloading pallets of materials  into steel storage racks in the warehouse section of the facility on the north side of zone (aisleway) 11A where steel shelving units had not been periodically inspected for corrosion, structural damage, or overloading.  Column leg post 6 had structural damage with pallets of materials being stored in the storage racks.   Column post 4 was out of alignment creating an unstable storage rack unit.  (g)  Employees loading / unloading pallets of materials  into steel storage racks in the warehouse section of the facility on the north side of zone (aisleway) 11B where steel shelving units had not been periodically inspected for corrosion, structural damage, or overloading.  Pallets of materials were being supported by structurally damage bracing between column posts 5 and 6.  Column post 7 was structurally damaged approximately 2 inches below the storage rack bracing.  Column posts 3, 4, and 6 were out of alignment creating an unstable storage rack unit.      Among other methods, one feasible and acceptable method to correct this hazard is: Develop and implement a structural integrity program to periodically inspect storage racks as recommended by the Rack Manufacturers Institute Specification for the Design, Testing, and Utilization of Industrial Steel Storage Racks 1997 Edition, Section 1.4.1
