On or about August 10, 2022, 14068 CR 455, Clermont, Florida, an employee/ the operator of a broom tractor was reaching into the engine compartment of the tractor with his bare hand in an attempt to shut off the engine my means of disconnecting/ unplugging a switch, due to the fact that the key ignition switch at the operators seat was inoperable, during which time the employer had failed to initiate and maintain safety, health, and accident prevention programs, as may be necessary to comply with this part/ construction activities; therefore exposing the employee to amputation type hazards from moving/ rotating parts such as the engine/ radiator cooling fan blades and related belts and pulleys and third degree burns/162F type hazards from engine/ engine exhaust.
