29 CFR 1926.502(d)(21):Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: Worksite located at 201 E. 133rd St., Kansas City, Missouri- Employee who is subject to fall hazards had failed to properly inspect the fall protection equipment to look for signs of deterioration, had he done it, he should have known to take the self-retracting lanyard out of service due to the deteriorating components on the strap.
