29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) Sylvania Senior Center doing business as Sylvania Community Services Center, Inc. located at 7140 Sylvania Ave. Sylvania, OH 43560: On or about February 2, 2023, the employer failed to establish an energy control program consisting of energy control procedures and employee training prior to an employee performing servicing and maintenance on energized equipment such as but not limited to a walk behind snow thrower. Without the benefit of an energy control program, an employee accessing the machine's augers/shoot to clear snow jams while the equipment was energized was exposed to caught in hazards.

Employees were exposed to hazards associated with using chemicals such as, but not limited to, Kaivac cleaning and rinse agent product and KSS bowl cleaners which can cause irritation to the skin, eyes and throat.
