29 CFR  1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.   a). On or about January 30, 2023, at the Lake Baldwin VA Hospital, 5201 Raymond Street, Orlando, FL, 32803, the employer did not utilize of a fall arrest system or guardrails after removing a cover from a 4' x 4' hole on the roof, exposing an employee to a 22 foot fall hazard.  Fall protection was not provided.
