29 CFR 1926.200(g)(1): Construction areas were not posted with legible traffic signs at points of hazards. On or about February 6, 2023, and at times prior thereto, employees were laying a water main in a trench that was over one hundred and fifty feet in length without the use of a protective system or traffic signs and warnings, exposing the employees to the hazard of being struck-by vehicular traffic.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. On or about February 6, 2023, and at times prior thereto, employees were installing a new water line in a trench that was over five feet deep without the competent person performing an inspection, exposing employees to a cave-in hazard.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about February 6, 2023, and at times prior thereto, employees were installing pipe in a trench that was over five feet deep without the use of a protective system, exposing the employees to a cave-in hazard.
