29 CFR  1910.215(b)(5): The employer failed to keep the maximum angular exposure of the grinding wheel periphery and sides for safety guards used on surface grinding machines which employ the wheel periphery under 150 deg. This exposure shall begin at a point not less than 15 deg. below the horizontal plane of the wheel spindle.  On or about February 8, 2023 the employer failed to maintain a guard for the front and both sides of the abrasive wheel on the grinding wheel station that maintained an exposure of 150 degrees or less. The lack of guarding around the wheel, especially around the areas not used for grinding, exposed employees using the grinding wheel to potential nip, entanglement, and catch points that can lead to injury caused by the abrasive wheel itself.

29 CFR  1910.219(c)(2)(i): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, were not protected by the employer using a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires.  On or about February 8, 2023, the employer failed to install any guards or enclosings on the horizontal shaft of the grinding wheel station that rotates when in operation. The moving shaft is open to employees using the grinding wheel, exposing them to entanglement and caught-by hazards.

29 CFR  1910.219(d)(1):The employer failed to ensure pulleys, any parts of which are seven (7) feet or less from the floor or working platform, were guarded in accordance with the standards specified in this section.  On or about February 8, 2023, the employer failed to install any guards or enclosings on the pulley system of the grinding wheel station that turns the shaft and abrasive wheel. The lack of guarding around the pulley system exposed employees using the grinding wheel station to potential pinch, entanglement, and caught-by hazards that can lead to injury caused by the system not being enclosed.

29 CFR  1910.219(e)(3)(i):Vertical and inclined belts failed to be be enclosed by a guard conforming to standards of this section.  On or about February 8, 2023, the employer failed to install guards or an enclosure for the belt of the pulley system that is attached to the motor on the grinding wheel station, exposing employees using the grinding wheel to caught-by hazards, nip points, and entanglement from the rotating belt.
