29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) On or before 2/8/2023, the employer did not ensure that Tank 9 was guarded effectively. The top of the tank was left open while the rotomix and side sweep mixing blades were in operation. Employees added ingredients to the tank as these parts were moving. This condition exposed employees to moving parts.  (b) On or before 2/8/2023, the employer did not ensure that Tank 7 was guarded effectively. The top of the tank was left open while the rotomix and side sweep mixing blades were in operation. Employees added ingredients to the tank as these parts were moving. This condition exposed employees to moving parts.  (c) On or before 2/8/2023, the employer did not ensure that Tank 5 was guarded effectively. The top of the tank was left open while the propeller and side sweep mixing blades were in operation. Employees added ingredients to the tank as these parts were moving. This condition exposed employees to moving parts.
