29 CFR  1926.416(a)(1): The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work.   Location - 1035 Main St. Haverhill, MA.  On or about 2/9/2023, employees was not protected while working in the direct vicinity energized electrical circuits.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Location - 1035 Main St. Haverhill, MA.  On or about 2/9/2023, employees performing solar panel installation on a residential roof were not protected from falls of greater than 9 feet to the ground below.
