Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems.  a. On or about 02/08/2023, at the jobsite located at 11785 Shirburn Circle, Parrish FL 34219, employees were exposed to fall hazards of approximately 9 to 24 feet, in that, fall protection was not used while engaged in roofing activities while standing and working on the roof.

On or about 02/08/2023, at the jobsite located at 11785 Shirburn Circle, Parrish FL 34219, employees were exposed to fall hazards of approximately 10 feet, in that, the extension ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.
