The Dayton 5-Speed 12.5" drill press (Model 3Z993B) was not provided with machine guarding for the rotating chuck to protect the operator(s) and other employee(s) in the power supply area. C. Machine operator(s) were exposed to amputation and caught-in hazards in the surface mount machine area. The motor positioning system of the Universal Instruments Corporation surface mount machine (S/N: 10003803) was not provided with an affixed guard to prevent accidental contact during operating cycle(s).
