29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employee(s) engaged in production activities in the surface mount machine and power supply areas of the facility were exposed to amputation, caught-in, and struck-by hazards in the following instances: A. Material handler(s) and other maintenance personnel were exposed to struck-by hazards in the power supply area of the facility. The Guardian Power 16-Speed heavy duty drill press (Model FDM-58-I6S) was not provided with machine guarding for the rotating chuck to protect the operator(s) and other employee(s) in the power supply area. B. Material handler(s) and other maintenance personnel were exposed to struck-by hazards in the power supply area of the facility. The Dayton 5-Speed 12.5" drill press (Model 3Z993B) was not provided with machine guarding for the rotating chuck to protect the operator(s) and other employee(s) in the power supply area. C. Machine operator(s) were exposed to amputation and caught-in hazards in the surface mount machine area. The motor positioning system of the Universal Instruments Corporation surface mount machine (S/N: 10003803) was not provided with an affixed guard to prevent accidental contact during operating cycle(s).

29 CFR 1910.305(g)(1)(iv)(D): Flexible cords and cables were used where attached to building surfaces: Employee(s) are exposed to electrical shock hazards. Employee(s) engaged in production activities in the surface mount machine area were exposed to electrical shock hazards in that flexible cords are used to power fans and other equipment while attached to building surfaces. Flexible extension cords powering fans and a motor speed control unit were being used while permanently attached to the building structure above and adjacent to several soldering areas within the facility.
