29 CFR  1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck:  a) Precision Truss LLC located at 7036 Ganges-Five Points Rd. Shiloh OH 44878: On or about February 13, 2023, the employer did not ensure employees were trained to operate a powered industrial truck through completion of training and evaluation prior to operating powered industrial trucks. In this condition, untrained operators of the Toyota, Caterpillar, and Komatsu powered industrial trucks, as well as other employees performing work tasks in the area, were exposed to struck by and crushing hazards.

29 CFR  1904.4(a):The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent.    a) Precision Truss LLC located at 7036 Ganges-Five Points Rd. Shiloh, OH 44878: On or about November 2022, the employer failed to maintain OSHA 300 logs for a recordable injury that occurred when an employee suffered a fractured foot after being struck by a powered industrial truck.
