29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a)  On or about February 16, 2023, at the Southwest corner of the building were employees were exposed to a fall hazard greater than 6 feet when installing exhaust fans on the roof without a fall protection system.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  a)  On or about February 16, 2023 at the Southwest corner of the building, where employees installing exhaust fans on a roof were exposed to a fall hazard when using a ladder to access the roof that was not extended 3 feet over the roof's edge.
