29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:   On or about February 16, 2023, employees engaged in commercial construction operations were using the GENIE S45 Aerial Lift (Platform) and exposed to fall hazards in excess of 10-ft. for not using fall protection while working from an aerial lift.

29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area.  On or about February 16, 2023, employee engaged in commercial construction operations using the GENIE S45 Aerial Lift (Platform) were exposed to fall hazards in excess of 10-ft. due to inadequacies in the knowledge and skills required for the safe operation of an aerial lift. Employees did not use any fall protection while in the basket.
