29 CFR  1910.138(a):The employer did not select and require employees to use appropriate hand protection when employees hands were exposed to hazardous condition(s)  a) Production Area; On or about February 16, 2023 and at times prior, employees were exposed chemical hazards, in that chemical resistant gloves were not provided to employees who hand fill industrial glues such as but not limited to Chemence SI5 Cyanoacrylate Adhesive and Chemence SI1500. The safety data sheets for both Chemence SI5 Cyanoacrylate Adhesive and ChemenceSI1500 indicated chemical resistant gloves are need to work with this chemical.

29 CFR  1910.212(a)(3)(ii): The  point of operation of machines whose operation exposed an employee to injury, was not guarded:  a) BS4 Alloyd Machine (Model 6S-CBE Serial 79-7); On or about February 16, 2023 and at times prior, employees were exposed to crush hazards, in that the point pf operation were employees place product into a mold on a rotating platform goes into the machine for further packaging contained approximately 3 inch gap. Once the product rotates into the machine a hydraulic press is activated to place a cardboard backing onto the product.

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical  a) Production Area; On or about February 16, 2023 and at times prior, employees were exposed to chemical hazards, in that a 50 gallon drum contained a label for Heptane Lo Arom Comm, when the 50-gallon drum actually contained Chemence AC11 (Liquid and Pump Spray). The 50-gallon drum did not contain a label indicating the contents and physical and health hazards of the drum at the time of the inspection.
