29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a. On the roof of a temporary residential worksite, at 7102 N Pearl Street, Jacksonville, Florida 32208: On or about February 16, 2023, the employer exposed three employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employees, while using pneumatic nail guns to nail down sheets of plywood and shingles the roof.  b. On the roof of a temporary residential worksite, at 7102 N Pearl Street, Jacksonville, Florida 32208: On or about February 16, 2023, the employer exposed two employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employees while using saws to cut sheets of plywood and 1 x 6's.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a.  On a 5:12 pitched roof of a temporary residential worksite, at 7102 N Pearl Street, in Jacksonville, Florida 32208: On or about February 16, 2023, the employer exposed three employees to fall hazards ranging from 8 feet to 15 feet 3-inches, in that fall protection was not utilized by employees while sheathing the roof.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.   a. Front of home, at a temporary residential worksite located at 7102 N Pearl Street, Jacksonville, Florida 32208:  On or about February 16,  2023, the employer exposed three employees to fall hazards, in that, the 16-foot extension ladder was not extended 3 feet above the landing.
