29 CFR  1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift.  On February 16, 2023, an employee elevated by JLG man-lift without a fall arrest system while performing construction activities on a commercial building was exposed to ejection and fall hazards of approximately 15 feet to the ground below.

29 CFR  1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous.   On February 16, 2023, employees engaged in construction activities utilized an extension cord with the missing ground pin to energize the painting machine exposing them to electrical shock hazards.
