29 CFR  1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   a) Work site - On or about November 27, 2022 and at times prior thereto, an employee was performing siding work from a scaffold platform approximately 12 feet high without the use of fall protection.  The employee fell from the scaffold resulting in a hospitalization for injuries to the right arm, right leg, and head.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:  a) Work site - On or about February 9, 2023 and at times prior thereto, the employer did not report an incident, resulting in hospitalization, that occurred on November 27, 2022.  The incident was referred to the area office by EMS following a second unrelated incident in the same area.
