A) Super Foodtown of Red Bank, 362 Broad Street RED BANK, NJ 07701: Employees that operated a Yale high low pallet jack, a Hyster Model W40Z, and Yale Model MPB040-EN24T2748 electric pallet jacks were not evaluated every three years on or about 2/22/2023.   NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
