1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Production Department- On or about February 18, 2023, employees were exposed to machine hazards associated with moving parts while webbing paper on the printing presses,  including the Goss Sunday 2000 Press and Heidelberg Sunday 2001 Press. The employer did not implement energy control application steps as the machine was not shut down or turned off to perform servicing tasks [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from moving equipment and pinch points.  b) Production Department- On or about March 6, 2023, employees were exposed to machine hazards associated with moving parts while cleaning rollers and the inside of the Color Units on the printing presses,  including the Goss Sunday 2000 Press and Heidelberg Sunday 2001 Press. The employer did not implement energy control application steps as the machine was not shut down or turned off to perform servicing tasks [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from moving equipment and pinch points.  c) Production Department- On or about March 6, 2023, employees were exposed to machine hazards associated with moving parts while changing rollers on the printing presses,  including the Goss Sunday 2000 Press, Heidelberg Sunday 2001 Press, and the Mini Presses. The employer did not implement energy control application steps as the machine was not shut down or turned off to perform servicing tasks [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from moving equipment and pinch points.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours, either by telephone or in person to the OSHA Area Office that is nearest to the site of the incident, by telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), or by electronic submission using the reporting application located on OSHA's public web site at www.osha.gov:     a) The employer did not report the in-patient hospitalization or amputation injury that was the result of a work-related incident that occurred on or about February 18, 2023, to OSHA within twenty-four (24) hours. The injury was reported to OSHA on February 20, 2023.  No abatement certification or documentation is required for this item.  Specialty Promotions, Inc. dba Specialty Printing Communication was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1904.39(a)(2), which was contained in OSHA inspection number 1512692, citation number 1, item number 1, and was affirmed as a final order on May 25, 2021, with respect to a workplace located at 6019 W. Howard St., in Niles, Illinois, 60714.

29 CFR 1910.147(c)(6)(i):  The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a) Pressroom - On or about March 16, 2023, the employer did not ensure that periodic inspections of the energy control procedures were conducted at least annually to ensure that the procedures and requirements of the energy control program were properly implemented and followed by employees while performing maintenance and servicing tasks on equipment, including the Goss Sunday 2000 Press, Heidelberg Sunday 2001 Press, and the Mini Presses.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
