29 CFR  1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:   Employees were exposed to struck-by hazards.  Employees operating the Clark Forklift, model number C500-S80, serial number 585-0036-7419KOF, were exposed to struck-by hazards.   The seat belt was removed during maintenance activity and was not reinstalled.
