29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation, or the loss of an eye: a) Phillips Gourmet, Inc., located at 124 Old Kennet Road, Kennett Square, PA 19348 - On or about February 18, 2023, an employee suffered and amputation to a finger. The employer was notified of the incident on February 18, 2023. The employer failed to report amputation to the Occupational Safety and Health Administration until February 21, 2023.
