29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  On February 17, 2023, at approximately 9:30AM, an employee was operating a flare valve when he suffered an injury that resulted in a hospitalization. The hospitalization was not reported within 24 hours to the local Area Offices of the Occupational Safety and Health Administration. The incident was finally reported on February 21, 2023, at approximately 1:25PM.
