29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about February 23, 2023, during the installation of siding material at a job site located at 7840 Wales Ave NW, N. Canton Ohio 44720; during the installation of roofing material, the employer did not ensure employees working on the ground were protected from possible danger of head injury from impact, or from falling or flying objects, falling debris from working surfaces of heights approximately 7.6 feet.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about February 23, 2023, during the installation of siding material at a job site located at 7840 Wales Ave NW, N. Canton Ohio 44720; the employer did not ensure employees installing siding material from the roof of a commercial party center facility were protected from a fall of approximately 7.6 feet by utilizing a fall protection system.
