29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) On or about February 24, 2023, at 404 Gulf Boulevard, Indian Rocks Beach, 33785, employees were allowed to work on ground level without head protection, during which time an excavator was being operated in the vicinity and was used to demolish the existing building structure, exposing employees to struck-by and crush-by hazards.

29 CFR  1926.501(a)(2):Employees were allowed to work on surfaces which did not have the requisite strength and structural integrity:  a) On or about February 24, 2023, at 404 Gulf Boulevard, Indian Rocks Beach, 33785, employees were allowed to work on surfaces such as the roof of a partially torn building and were expose to a structural collapse due to the roof not having structural integrity.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a) On or about February 24, 2023, at 404 Gulf Boulevard, Indian Rocks Beach, 33785, employees were allowed to work on surfaces such as the roof of a partially torn building without fall protection and were exposed to a fall hazard of approximately 40 feet.
