29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. On or about February 21, 2023, the employer did not ensure that periodic inspections of energy control procedures were conducted at least annually to identify and correct any deviations or inadequacies. Employees were exposed to amputation hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedure) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6): On or about February 21, 2023, the employer did not ensure that employees implemented energy control application steps prior to allowing them to clean the 5FT Roller by hand in the Ram Group Department. The 5FT Roller was not shut down or locked out prior to employees engaging in this servicing and maintenance activity, exposing them to pinch/nip point hazards and amputation injuries. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. On or about February 21, 2023, the employer did not ensure that employees who performed service and maintenance tasks on the 5FT Roller Machine were adequately trained on the means and methods needed to effectively isolate and control hazardous energy sources. Employees were exposed to pinch and nip point hazards of equipment in operation which could result in caught-in, crushing and amputation injuries while cleaning the rollers. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
