29 CFR  1910.252(b)(3): Employees exposed to the hazards created by welding, cutting, or brazing operations were not protected by personal protective equipment in accordance with the requirements of 29 CFR 1910.132:  In the facility at 75 Blackstone River Road, Worcester, MA: On or about February 27, 2023, employees were exposed to molten metal sparks while operating the resistance welding machines and the employer had not provided adequate personal protective equipment to include gloves, arm guards and/or fire rated clothing to employees operating the resistance welders.  An employee suffered skin burns from exposure to flying metal sparks.

Worcester, MA on February 27, 2023, the employer did not ensure that the Safety Data Sheets binder remained available during renovations activities causing a period of time when employee lacked access to SDS's for workplace hazardous chemicals.  These hazardous chemicals included but were not limited to ALPET D2 Surface Sanitizer Wipes, Citrex HD Heavy Duty Industrial Degreaser and Zenlektrix Contact Cleaner.
