29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers. (a) On or about February 23, 2023, an employee installing insulation in a chimney was exposed to a fall, in-that the floor hole was covered with cement board which did not support the weight of an employee working on the cement board. This exposed the employee to a fall hazard of 4.5 feet.
