29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   (a)Job site: 524 5th Street Tarentum, PA, on or about February 24th, 2023 employees were not protected from fall hazards while installing roof patches on a flat roof approximately 85 feet in length by 43 feet in width and 25 feet above ground level. Employees were working approximately 5 feet from the roof edge.

(a) 524 5th Street, Tarentum, PA On or about February 24th, 2023, the employer did not maintain the  fall protection certification training record for employees exposed to fall hazards of approximately 25 feet while installing roof patches on a flat roof of an apartment building.
