29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  A) 40 Pondfield Road, Bronxville - NY : Employees were exposed to fall hazards of approximately 12  feet while working on a fabricated framed scaffold that was not fully plank, on or about January 13th, 2023.

29 CFR  1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access:  A) 40 Pondfield Road, Bronxville - NY : Employees were exposed to fall hazards of approximately 16 1/2  feet while using an extension ladder to access landing. The extension ladder that was used, did not extend at least 3 feet above the landing, on or about January 13th, 2023.
