29 CFR  1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  On or about February 28, 2023, employees engaged in roofing activities were exposed to fall hazards while using a ladder that did not extend at least 3-feet above the roof's landing.
