29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Exterior Front of Single Family Frame Home: Employees on the ground level, engaged in cleaning and removing roofing debris, were exposed to falling objects and were not wearing protective helmets. The employer did not ensure that employees were wearing protective helmets, thus exposing employees to a struck by hazard from falling objects, on or about 3/1/23.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Exterior (South Facing) of Single Family Frame Home: Employees engaged in roofing work were required to access the roof from an aluminum extension ladder, and the ladder did not extend at least 3 feet above the roof's landing. The employer did not ensure that an extension ladder used to access the roof, was extended to at least 3 feet above the upper landing surface, on or about 3/1/23.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) Exterior (East Facing) of Single Family Frame Home: Employees were required to remove old shingles and install new roofing material on a residential home, on a 12 to 12 pitched roof. Employees were exposed to a fall hazard of over 23 feet above the lower surface, without the use of a fall arrest system, or other means of fall protection, on or about 3/1/23.
