29 CFR  1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:   On or about March 1, 2023, employees working in areas where there is a possible danger from head injury while completing siding work at a residential home located at 225 Windflower Drive, Chardon, OH 44024, were not protected by protective helmets.

29 CFR  1926.451(g)(1):Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level:   On or about March 1, 2023, the employer did not ensure an employee on a ladder jack scaffold more than 10 feet above the lower levels while completing siding work on a residential home located at 225 Windflower Drive, Chardon, OH 44024, was protected from falling to lower level.
