(a) Jomax Construction Co., Inc., 21496 Co Rd 47, LA SALLE, CO 80645: On or near March 1, 2023, and at times prior, employees were exposed to crushing hazards when the employer did not prohibit use of the 583K Pipelayer (Unit C-165) after a kink in the wire rope was identified.
