29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a. On the roof of a temporary residential worksite, located at 6720 Sunset Drive, in Jacksonville, Florida 32208: On or about March 2, 2023, the employer exposed five employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employee, while using pneumatic nail guns to install roofing shingles.  HYR Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1613959, citation number 01, item number 001 and was affirmed as a final order on December 21, 2022, with respect to a workplace located at 2055 Haines Street, Jacksonville Firemen's Credit Union, Jacksonville, FL 32220.

29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   a.  On a 6:12 pitched roof of a residential worksite,  located at 6720 Sunset Drive, Jacksonville, Florida 32208: On or about March 2, 2023, the employer exposed five employees to fall hazards ranging from 10 feet to 16-foot, 5-inches, in that fall protection was not utilized by employees while installing roof shingles.  HYR Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10) which was contained in OSHA inspection number 1613959, citation number 01, item number 002 and was affirmed as a final order on December 21, 2022, with respect to a workplace located at 2055 Haines Street, Jacksonville Firemen's Credit Union, Jacksonville, FL 32220.

On the right side of a residential home, located at 6720 Sunset Drive, Jacksonville, Florida 32208: On or about March 2, 2023, the employer exposed employees to fall hazards, in that employees had taken apart a 32-foot extension ladder and only using one section to access and egress to the roof, exposing employees to fall hazards.
