29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a). 1800 A Sarsota, Parkway, Conyers, GA 30013; On or about March 3, 2023, and at times prior thereto, in the Dry End production area, employees were exposed to caught-in hazards while operating the unguarded Auto line machine when paper jammed, the employee placed hands inside to pull paper out of the machine and nipped his fingers creating an amputation hazard.

1910.147(c)(7)(i)(A)  Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  a). 1800 A Sarsota, Parkway, Conyers, GA 30013; On or about March 3, 2023, at the L&W Autoline 300 machine the employer did not ensure an employee was adequately trained to utilized the lockout tagout procedures to controlling potentially hazardous energy, such as but not limited to electrical, pneumatic, hydraulic, gravitational, and mechanical, exposing the employee to a point of operation hazard resulting from unexpected start-up while reaching into the machine to unjam paper.
