At a jobsite located at 2191 S 135th W, Unit # 1067, Wichita, KS 67235 employees conducting residential roofing activities were exposed to serious fall hazards of approximately 10 feet from the ceiling joists to the ground below in that fall protection was not provided.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At the jobsite located at 2191 S 135th W, Unit # 1067, Wichita, KS 67235 employees engaged in residential roofing activities were exposed to fall hazards when accessing an extension ladder: 1) An aluminum Gorilla extension ladder utilized at the residential building was 11 feet from the eave of the residence to the ground Employees accessed and exited the roof utilizing the portable ladder and it did not extend at least 3 feet above the upper landing and was not secured to the structure.
