29 CFR  1910.179(j)(2): Frequent inspection. The following items shall be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections. All deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard.  On or about February 26, 2023, at an establishment located at 1600 Ely Street in Marinette, Wisconsin, employees were utilizing an overhead crane that had not been inspected prior to the shift start exposing employees to overhead and struck by hazards.

On or about February 26, 2023, at an establishment located at 1600 Ely Street in Marinette, Wisconsin, employees were utilizing an overhead crane while not staying clear of the load being exposed to overhead and struck by hazards.

29 CFR  1915.111(a): All gear and equipment provided by the employer for rigging and materials handling shall be inspected before each shift and when necessary, at intervals during its use to ensure that it is safe. Defective gear shall be removed and repaired or replaced before further use.  On or about February 26, 2023, at an establishment located at 1600 Ely Street in Marinette, Wisconsin, an employee was utilizing an overhead crane with a pad eye used for rigging and the pad eye had not been inspected when necessary before a lift, exposing employees to struck by hazards.

On or about February 26, 2023, at an establishment located at 1600 Ely Street in Marinette, Wisconsin, employees were utilizing an overhead crane to fair steel with a pad eye.  The pad eye was not safely rigged because it was not fully welded exposing employees to struck by hazards.

On or about February 26, 2023, at an establishment located at 1600 Ely Street in Marinette, Wisconsin, an employee was utilizing an overhead crane and the operators did not have an operator's manual available for the employee in the language of certification.
