29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by the use of a guardrail system, safety net system, personal fall arrest system, warning line system, or a safety monitoring system.   a) On or about March 7, 2023, the controlling contractor, Dodd Roofing Contractors, LLC, did not ensure employees were protected from falls hazards of approximately fourteen (14) feet and up to approximately twenty-four (24) feet while performing roofing activities.    Note)  Certification and documentation of corrective actions are required for this item.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) On or about March 7, 2023, the controlling contractor, Dodd Roofing Contractors, LLC, did not ensure proper set-up and use of the ladders used by employees to access the roof.  The ladder access on the west side of the front canopy was placed with the top rung even with the eave.  Note) Certification of corrective action is required for this item.
