OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck-by  hazards:  A) At the facility located at 25947 Hwy 23 in Collins, GA: On or about March 7, 2023, the employer exposed employees to struck-by hazards, in that the back-up alarm on a Hyundai front-end loader was not functioning and the passenger side mirror was missing.   B) At the facility located at 25947 Hwy 23 in Collins, GA: On or about March 7, 2023, the employer exposed employees to struck-by hazards, in that the horn on a Volvo front-end loader was not functioning.

29 CFR 1910.27(a): The employer did not ensure that scaffolds used in general industry meet the requirements in 29 CFR part 1926, subpart L in that 29 CFR 1926.450(a)(6): A scaffold was not designed by a qualified person and was not constructed and loaded in accordance with that design:  A) By Reclaim conveyor located at 25947 GA Hwy 23, Collins GA:  On or about 3/7/2023, the employer exposed an employee to a fall hazard in that a Volvo front-end loader that was not designed to be as a scaffold was used to lift the employee in the air to perform work.
