Facility Wide- On or about March 7, 2023, the employer did not ensure that employee(s) who operate powered industrial vehicles, including the Crown Reach Truck, Model: RM 6000 Series, Serial No. 1A531212, Crown Forklift, Model SC 5200 Serial No. 10202632, Crown Order Picker, Model: SP 4000 Series, Serial No. 1A531212, Crown Standup Forklift, Model: RC 5500, Serial No. 1A600015, and the Crown Clamp Truck, Model: SC 5200 Serial No. 10260397, were properly trained, evaluated and certified to perform the tasks in a safe manner, thereby exposing employees to struck-by hazards.
