29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used and the employer allowed cross braces to be used as a means of access: a) Job site, 8131 Ohio River Boulevard, Pittsburgh, PA: On or about March 8, during masonry activities, the employer did not ensure that employees had a safe way to get up/down from the scaffolding in that employees were climbing cross bracing and walkthrough scaffold section frames to do so, exposing them to hazards such as falls from heights of 10 feet or greater.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: a) Job site, 8131 Ohio River Boulevard, Pittsburgh, PA: On or about March 8, while working on scaffolding to perform masonry work, the employer did not ensure that appropriate fall protection was in use at the site, exposing workers of fall hazards of up to approximately 12 1/2 feet or so.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation: a) Job site, 8131 Ohio River Boulevard, Pittsburgh, PA: On or about March 8, during masonry activities, the employer did not ensure that scaffolding was properly set up in that in some places base plates rested on concrete block that was filled with grout, in other areas the baseplates did not rest completely on the mudsills that sat on the ground or completely above and on the built up layers of wood used to accomodate uneven ground.
